Many properties require a Certificate of Insurance (COI) before installation or service work can begin. ButterflyMX makes it simple to request and receive the documentation you need. Below you’ll find guidance on what information to provide and what to expect once your request is submitted.
Q: How can I request a Certificate of Insurance from ButterflyMX?
A: To proceed with your request, please send us the following information:
Certificate Holder: The name and address of the certificate holder.
Description of Operations: Any specific requirements or details about the services, operations, or activities the insurance should cover.
Coverage Limits: The coverage limits required, especially if they differ from our standard certificate.
Q: What if my company has its own COI template?
A: If you have a specific COI template or format that needs to be used, please forward it to us along with the requested information. This ensures the certificate fully meets your requirements.
Q: How long does it take to receive the COI?
A: Once we receive your request and all required details, we will forward it to our insurance partner, Fieldstone. They typically process and send the COI within 48 hours.
Q: Who should I contact to request a COI?
A: Please send your request and supporting information to invoices@butterflymx.com.
Note for Canadian customers: Please note that ButterflyMX does not perform any on-site work; the installer is responsible for providing the Workers' Compensation Board (WCB) information.
